Checklist Before Leaving Home

  • Do not eat or drink anything in the morning or take any medication unless you have been given special instructions stating otherwise.
  • Do not wear jewelry, watches, makeup or nail polish. If you wear contact lenses, please bring a case with you as well as glasses.
  • Wear comfortable clothing that you can change easily and that will not bind the site of your surgery.
  • Leave valuables and jewelry at home since ASCS can not be responsible for lost articles.
  • YOU MUST HAVE A RESPONSIBLE ADULT IN THE BUILDING THE ENTIRE TIME YOU ARE IN SURGERY.
  • Make sure you have a ride home and arrange for someone to stay with you and your children for 24 hours following your surgery as you may feel drowsy for that period of time.
  • In case of an emergency we need someone that is able to make decisions for you.
  • Minors (under age 18) must be accompanied by a parent or legal guardian. •Please bring any legal documents for proof of guardianship or Power of Attorney
  • If you are a diabetic, we will take your blood sugar upon arrival. If you take insulin, bring it with you.
  • If you use inhalers, please bring them with you.
  • If you are scheduled for orthopedic or podiatry surgery, please notify your nurse. if you have a walker, wheelchair, crutches, or post op boot or shoe. You will need these items post-operatively.
  •  Remember to bring your insurance card, drivers license, and Medicare card (if applicable).
  • Please be prepared to pay your insurance co-pay or other deductible prior to your surgery.
  • If you do not speak English, arrange for an interpreter to accompany you to the Center. This person should remain until you are discharged,
